- In what town/city is the patient located?
- Where is the ambulance needed?
- What is the caller’s and/or patient’s telephone number?
- What is the medical problem with the patient?
- What is the approximate age of the patient?
- Is the patient awake (yes or no)?
- Is the patient breathing (yes or no)?
